State Motor Vehicle Form

Regulations
  1. Vehicle operators must have a valid driver's license on file and must be an employee of New Jersey City University.
  2. Vehicle use is limited to "Official University" business.
  3. Vehicle operators must obey all motor vehicle regulations and is responsible for all violations.
  4. All accidents and/or damages must be promptly reported to the Public Safety Department Shift Supervisor upon arrival to the University along with a police report.
  5. If gas/and or oil are put into the vehicle, you must write the license plate # of that vehicle at the top of the receipt and submit with all paperwork to the Public Safety Office.
  6. When you return back to the University, all garbage must be removed from the vehicle and the vehicle must be parked in Lot #3 (West Campus). No Vehicles are to be parked in the G-SUB Garbage / Lot #1, Lot #4 or on the immediate campus ground.
  7. All information on this form must be completed and signed by the driver.
  8. Vehicle must be inspected prior to put in service.
  9. No more than 10 passengers (9 Passengers + 1 Driver) are allowed in the 15 Passenger Vans.
