Easy To Follow Instructions:
After approx. the 5th - 7th week of class, email or give this link to your students, to access the online evaluation form.
(It is important that you instruct your students to enter your
Full Name,
your
Email Address and the
Course Number, in the appropriate form fields.
These three items are required and must be completed so we can properly match up the results to the appropriate faculty member.)
- Ask your students to complete the online evaluation form by a specific date (within about one week's time).

After that date, submit a web support ticket by clicking here:
Online Web Support requesting your
CTL Mid-Semester Evaluation Report. (Log in with your GothicNet Username and GothicNet Password. After logging in, click on the link that says
"Submit Ticket" in order to send your request.)
Please provide the following along with your request:
1. Complete all of the applicable fields in the "Support Ticket" system.
(Optional Fields can be left blank and are not needed.)
2. Choose
"Other - Include Description Below" for the first required field.
3. As the Subject for your "Support Ticket," please enter:
"Request for CTL Mid Semester Evaluation Report"
4. In the
"Message" area (located towards the bottom of the form), please include the
approximate dates during which your students would have completed the online evaluation reports (i.e.
Report Dates: 01/01/2010 through 01/22/2010), as well as the
4 Digit NJCU Course Number for each course that your students would have completed the online evaluation report for during this period.
Then, within just a few days, you will receive a .CSV file (Excel style spread sheet) sent to you by email with all of the responses and comments that were provided by your students.
