Class Absences & Medical Withdrawals
 
 
General Information
 
The Health & Wellness Center (H&WC) does not routinely provide "medical excuse" notes. Our " Verification of Visit"  form will be provided to students only if they are seen and evaluated in the H&WC and are deemed to be medically unable to attend class or participate in NJCU athletic practice sessions. 

It is not the intent of the H&WC to use this practice to exempt students from class responsibilities or to interfere with individual professor's requirements for class attendance or coursework assignments. It is ultimately the responsibility of the student to comply with each professor's course requirements and policy for missed classes.

Student's Responsibility
 
As noted above, each student is expected to fullfill the attendance requirements as determined by the course instructor. Whenever time is missed from classes, it is the responsibility of the student to notify the instructor and make their own arrangements to make up the assignments/work missed from class on an individual basis.
 
Extended Absences
 
Students who will be absent for an extended period of time  (greater than five consecutive days for medical reasons) should contact the Office of the Dean of Students at (201) 200-3525.  The Dean's office will notify the faculty, however the student remains responsible for fullfilling the course requirements.
 
Medical Withdrawals
 
If you find it necessary to withdraw from the University due to medical circumstances (i.e. hospitalization for an accident or illness), you must arrange an appointment with the Director of Counseling & Wellness Services by calling 201-200-3165. Be sure to indicate that you would like to withdraw from the University for medical reasons. You must provide appropriate supporting documentation at the time of the appointment.

Students who choose to appeal the Director’s decision should contact the Dean of Students at 201-200-3525 to request an appointment.

In addition to meeting with the Director for Counseling and Wellness Services, you must also officially withdraw from each of your courses at the Registrar’s Office , located at Hepburn 214 ( 201-200-3334).

If you are requesting a medical withdrawal after the semester is completed, you must make an appeal to the Dean of Arts & Sciences ( 201-200-3001).
