Entrance Health - General Information
In addition to providing health care, the HWC oversees the University health requirements for all entering students.
Each NJCU student MUST submit the
NJCU Entrance Health Record. (4 pages)
This information is used to establish a student's medical file and document compliance with state immunization laws. All information supplied is maintained in a CONFIDENTIAL manner and cannot be released to another party without the student's written consent. Forms can also be obtained from the Admissions Office or the Health and Wellness Center or downloaded above. Students may send or fax documentation signed by a licensed health care provider, a copy of a signed and authorized school record, or the completed NJCU Immunization form, signed and stamped by an authorized health care provider. Immunization Requirements:
New Jersey State Law requires all students, both undergraduate and graduate (born after 1956) to submit proof of immunity to Measles (2 doses), Mumps, and Rubella or file a valid exemption. Detailed information about the required immunizations
or valid exemptions can be found on page 3 and 4 of the
Entrance Health Record.
Effective Fall 2004, all new students ( who plan to live in campus housing) must present documented proof of one adult meningococcal meningitis vaccination (Menactra or Menomune) as a condition of residence at NJCU. Students who fail to submit this information will not be permitted to move into campus residential facilities.
The State of New Jersey requires that all full and part-time incoming freshmen, transfer, and graduate students be provided with information about meningitis and the availability and benefits of the meningococcal meningitis vaccine. Each student must be surveyed about their decision whether or not to receive the vaccination. This information and survey is located on page 4 of the
NJCU Entrance Health Record.
New Jersey Colleges and Universities have been informed that proof of immunization or immunity to
Hepatitis B will be required effective Fall 2008. At the present time, we have been notified that the Hepatitis B immunization requirement will apply to NEW students (as of Fall 2008) taking 12 credits or more (Undergraduate and Graduate Students)*.
We are awaiting the complete compliance guidelines from the New Jersey State Department of Health.
*
Some students in special programs (such as nursing) will have to complete this requirement, as well as any additional requirements set forth by their major program of study or internships.
Skin testing for exposure to Tuberculosis (TB) is not currently required for all students. We stronlgy advise, as a good health practice, that you consider having this screening test performed (Mantoux or PPD). As with other optional tests and vaccines, Tuberculin (TB) skin screening will be required for some majors, internships, or work experiences. Your will be notified by your individual programs, of this requirement. |
