Privacy & Confidentiality
 
 
All healthcare visits to the NJCU Health & Wellness Center (H&WC) are strictly confidential. Your medical chart begins with your NJCU Entrance Health form. Notations are made in your individual chart each time you receive services at the H&WC. Your H&WC medical chart is completely separate from any of your other university or academic records.

Patients 18 or older must sign a written consent form to release any information except in the following situations:

  • In the event of an extreme life-threatening emergency
  • Subpoenas or court orders
  • NJ State Health department reporting requirements

Parent/guardian notification occurs only when a student is under 18 years of age or when a student’s medical condition renders them unable to make a decision.

Medical records are retained for 10 years from the patient’s last date of care, after which records are destroyed by shredding in compliance with federal privacy regulations.

Data from immunization records are entered into our computer system and can be retained longer than 10 years.
 
 
  Rights and Responsibilities of Patients:
  • Patients are treated with respect, consideration, and dignity
  • Patients are assured personal privacy, including protection from access to their confidential information.
  • Patient disclosures and records are treated confidentially
  • Patients are provided, to the degree known, information concerning their diagnosis, treatment, and prognosis upon their request
  • Patients are given the opportunity to participate in decisions involving their health care, whenever possible
  • Patients have a right to competent, humane treatment, with appropriate respect for their opinions and beliefs in the process of receiving health care
  • Patients are responsible to give the health care provider complete and full information on their past and present health status and to cooperate in treatment planning and follow up. Responsibilities include asking questions of health care providers, making use of available information and education concerning their own health, and showing the same respect for health care personnel as they expect to receive.
  • Patients are responsible to express suggestions and grievances, whenever indicated, in order to assist the college health service in improving the quality of health care and education services.
