Below you will find appropriate forms to be completed for services available with OSS/Project Mentor.  Please read each form carefully to insure that you are requesting the appropriate service.
 
Office of Specialized Services Student Client Intake Form
 
Students registering or updating their contact and accommodation information should complete the client intake form.  Submit completed forms to Office of Specialized Services/Project Mentor office in P-343/P-340. 
 
 
Testing Accommodation Request
 
Students who are requesting testing accommodations, such as alternate testing facilities, extended time for testing, and/or use of computer with spell-check, must complete the testing accommodation request form. Testing Accommodation Request Forms must be submited up to two (2) weeks prior to the scheduled exam and returned to Ms. Jackson or Ms. Aitken. If you require testing accommodations for your exams and/or quizzes, you must complete this form for each request. Please fill in the grid below with all information regarding your test. We recognize that you may not be aware of your tests far in advance, but we wish to receive this information at least one to two weeks in advance.
Tests/quizzes are proctored by Project Mentor staff and any form of academic dishonesty will be reported to your instructor and/or the appropriate Dean. The University states that any violation of academic integrity may be subject to a penalty based on the infraction that may include a reprimand, reduction in grade, failing grade, suspension, or dismissal from the University.
Students are not permitted to have access to books or notes or any other materials (cell phone, internet access, etc.) during testing unless written notice is received directly from the instructor.
 
Books in Alternate Format 
 
Students who require textbooks or course books in Alternate Format (such as CD/E-text/Tape) must complete the request form and submit completed form to Ms. Jackson in P-340.  Requests may take several weeks to be fully processed, therefore please submit requests within the first two weeks of the semester.

Project Mentor Scholarship 

Students in the Project Mentor program are encouraged to apply for the annual Project Mentor Scholarship.  Click on the link above for the 2009-2010 Scholarship Application!
 
For scholarship consideration, you must:
  • Be a current NJCU student
  • Registered with Project Mentor
  • Have completed 20 or more credits at NJCU 
  • Posess a cumulative GPA of 2.5 or higher
  Application Deadline for the 2009-2010 Scholarship
is Thursday, October 15th!
