SICK LEAVE POLICY

ELIGIBILITY FOR SICK LEAVE

Employees are credited with one and one-quarter (1 ¼) days of sick leave for each month of employment for a total of fifteen (15) days annually for twelve month employees and a total of twelve and one-half (12 ½) days annually for ten month employees.

Unused sick leave is cumulative and may be used for personal illness. Sick leave may also be used in the event of the death of a member in the immediate family or in situations which require the short-term attendance of a member of the immediate family who is seriously ill.

REPORTING OF SICK LEAVE

For reasons of business necessity, absences due to illness or death in the family must be reported immediately to your supervisor. Should illness require an employee to be absent from work for five (5) consecutive working days or more, medical documentation must be submitted to the Human Resources Department after the fifth day of the absence.

MEDICAL LEAVE OF ABSENCE

When it becomes evident that a medical condition will require additional absence from work, a medical leave of absence (with or without pay) must be requested in writing and approved by the Department Head. Such requests require medical documentation stating the nature of the illness, prognosis and expected return to work date.
